Carlos Alcaraz advanced to the Barcelona Open second round with a 6-4, 6-2 win over qualifier Otto Virtanen, but raised alarms with a forearm injury scare, reporting sharp pain on forehands that prompted a medical timeout—his status remains uncertain ahead of Thursday's clay-court clash. The Spaniard, a two-time defending champion with a stellar 14-3 record here and home-crowd backing, rebounds from Sunday's Monte-Carlo final loss to Jannik Sinner on minimal rest. Tomas Machac, riding momentum from his Adelaide 2026 title and a gritty 2-6, 6-4, 6-1 upset of clay expert Sebastian Baez, holds an even 1-1 head-to-head edge over Alcaraz, including a prior hard-court win. Mild weather favors play, but Alcaraz's health drives trader consensus.
