Opposition from community health centers, the California Primary Care Association, and major medical groups has shaped trader views on Proposition 44, the November 2026 ballot measure requiring nonprofit Federally Qualified Health Centers to direct at least 90 percent of revenue to program services or face penalties. Analyses projecting up to $1.7 billion in first-year noncompliance costs, potential clinic closures, and shifts in patient care to higher-cost settings have reinforced skepticism. A lawsuit challenging the initiative’s qualification added procedural uncertainty earlier in the cycle. With the measure backed primarily by the SEIU-UHW union and facing coordinated institutional resistance just two months before the vote, current pricing reflects these structural and fiscal barriers more than early public polling.
