**Strong opposition from California community health centers and their associations, including lawsuits challenging the measure’s placement on the ballot, has anchored trader expectations that the 90% revenue-spending requirement will fail.** The initiative, backed by SEIU-UHW and qualified via petition signatures for the November 2026 ballot as Proposition 44, would impose penalties on federally qualified health centers falling short of the threshold for patient-care and mission-related expenses. Independent analyses project roughly $1.7 billion in first-year noncompliance costs, potential service reductions, and clinic closures, arguments amplified by clinic groups that contend the formula excludes legitimate overhead and patient-support spending. With three months until the vote, the 69.5% implied probability for rejection reflects these institutional barriers and fiscal concerns more than polling data.
