**Opposition from community health clinics, major medical associations, and a federal lawsuit filed by the California Primary Care Association has driven trader consensus toward rejection of Proposition 44.** The measure, qualified for the November 3, 2026 ballot by SEIU-UHW, would require federally qualified health centers to spend at least 90 percent of revenue on program services or face penalties potentially totaling $1.7 billion in the first year. Analyses project service reductions or clinic closures if enacted, while clinics argue the formula conflicts with federal rules and excludes key patient-care costs. Coordinated resistance, including over $33 million in opposition spending and ongoing legal challenges, outweighs union support and has produced the current 73 percent implied probability of failure. No major new developments have shifted the balance in recent weeks.
